NURSE'S OFFICE

Our school nurse is available to all Lincoln-Way students for health counseling and immediate care for illnesses and accidents occurring during school hours. All incoming freshmen and new students' physical forms are verified by our nurse for compliance with Illinois State Laws.

MEDICATION CARE

Medications may only be administered by our school nurse with appropriate forms on file in our school nurse's office. Students in need of daily or as needed medication require a doctor’s order on the permission form.

HEALTH FORMS

 REQUIREMENTS FOR INCOMING SENIORS

The Meningococcal vaccine is a Illinois Department of Public Health requirement for all high school seniors. Incoming senior students must submit proof of receiving two doses containing Meningococcal Conjugate Vaccine (MCV4). However, if the first dose was administered to a child who was 16 years of age or older, then only one dose is required. The documentation needs to be submitted to Nurse’s office prior to the start of the student’s senior school year.

REQUIREMENTS FOR INCOMING FRESHMEN

All freshman students are required to submit physical and dental forms prior to the beginning of the school year. The Certificate of Child Health and dental forms address all health requirements and immunizations for entry into 9th grade per the State of Illinois guidelines. These forms must be completed and signed by the student’s parent, physician, and dentist. Freshmen in Sports or band should use the Certificate of Child Health form to fulfill the physical requirements.
